- To File an Employment Complaint:
If you wish to file a complaint related to employment, you must contact your Agency’s EEO (Equal Employment Opportunity) Counselor within 45 days from the date of the alleged discriminatory act, event, or personnel action.

- To File a Program Complaint (Civil Rights Discrimination):
To file a Civil Rights program complaint of discrimination, you can:
- Complete the form online.
- Obtain the form at any USDA office.
- Call (866) 632-9992 to request the form.
- Write a letter including all the information requested in the form.

Michigan Civil Rights Protections
In addition to federal protections, Schoolcraft Memorial Hospital adheres to the Elliott-Larsen Civil Rights Act (Act 453 of 1976), which defines civil rights and prohibits discriminatory practices, policies, and customs in Michigan. This act specifically protects against discrimination based on:

- Weight
- Familial status
- Marital status
The act also preserves the confidentiality of records regarding arrest, detention, or other disposition without a conviction, and outlines the powers and duties of the civil rights commission and department, providing remedies and penalties.
