How do I qualify for home health care?

Home health care is a resource dedicated to those who lack accessibility to the care they need or have special requirements for their care.

Our requirements to qualify for home health care services include:

  • A physician’s order
  • A skilled need; for example, nursing, physical therapy, etc.
  • Being considered homebound, which means leaving home requires much forethought, planning, and/or assistance

How do I qualify for hospice care?

Hospice care is about more than comfort—it’s holistic care that supports the physical, emotional, and spiritual needs of individuals and their families as they navigate the end-of-life journey.
To be eligible for hospice services, patients must meet the following criteria:
  • A physician certifies a terminal illness with a life expectancy of six months or less if the illness follows its expected course
  • A caregiver or support system is available to help ensure safe care in the home

Are home health and hospice care covered by my insurance?

Medicare and most Medicare HMOs cover costs for qualifying individuals. Medicaid, Blue Cross Blue Shield, other insurances, and private pay are also acceptable payment options.
